THE STATE OF HEALTH IN POLAND
The average Health Index in 2023 was 63.6 %, 1.5 % higher than the first edition of the National Health Test of Poles, which was conducted in 2020, before the pandemic was announced. After 2021 and 2022, when the Health Index was lower than in 2020, there was a significant rebound in 2023. This means that Poles are following experts' recommendations for disease prevention and healthy lifestyles significantly better than during the pandemic, and minimally better than before the pandemic, and thus taking better care of their health.
CASE REPORT OF A PATIENT IN PRIMARY CARE
Jan Nowak, 42 years old. IT specialist in a large corporate company. He has been living alone since his parents death. Maintains good relationships with neighbors and colleagues at work. He has been obese since childhood. Currently BMI 36. Sedentary lifestyle, occasional physical activity.
A few weeks ago he tried to lose weight, he bought a gym membership. Due to rapid fatigue and a two-fold drop in glycemia, he gave up exercising. Interests: computer games and reading books.
He has been complaining of pain and dizziness for a week. A chronic wound was observed on the right foot.
CONDITIONS
• Networks (single, no children, sporadic contacts with friends, does not participate in team-building meetings at work)
• Socio-economic (salary - meets the man's basic needs, owns an apartment and a car)
• Cultural (a homebody, does not take advantage of the city's cultural offer: he does not go to the cinema, to exhibitions, or to restaurants)
• Environmental (big city; close to shops, primary care clinics, bus stop, pharmacy; city park near the house; high noise concentration)
• Health system – access to primary care clinics, specialist clinics and 2 hospitals located in the city
Hi! I notices that Jan's hobbies are more introverted. I was wondering if he had a good support system. Outside of the relationships he maintains at work, I feel like if we know how his support system we could get a better understanding of his psychosocial health.

Hello! Judging by the information provided, it seems Jan requires a lot of education regarding lifestyle changes such as diet and exercise that could help him manage his health problems. I his job a sedentary like his life style. and does he engaged in any purposful exersize? how does the sedentary life style affect his mental health?
Hello, I would like to know how he is psychologically, based on the information provided, it seems like he has had a lot stressors in his life, including the death of his parents, and not participating in the team-building meetings at work. He does not seem to go out very much and has sporadic contact with his friends. If he has given up on exercising too it might have taken a toll on his mental health.
How does the chronic wound on his right foot affect his walking? Does it cause him pain? I think it would be important to assess his foot for signs of infection and educate the patient on the importance of daily feet checks and skin care.
I think based on his current symptoms and state of health it would be a good idea to look at his diet and determine if that is causing any of the issues. Does he have any past medical history such as diabetes that would be causing these symptoms?
Hi! I would like to know more about his medical history other than his obesity. Does he have any diagnosed history such as diabetes, high blood pressure, high cholesterol, thyroid issues, depression, or anxiety? If he does have any of those comorbidities, how well or if at all is he controlling them? Is he taking any medications as well?
From the information provided in the case study, I would like to focus on dietary education for this patient. An individual who has been obese since childhood will need to make changes in many areas of life, not just physical activity. I would also try to determine a physical activity plan with the patient that is attainable, or a plan that can be gradually increased in intensity. In this case, were these factors considered?
I would like to know more about his dietary habits. I see that he does not go to restaurants, so does he cook his own meals and if so what does he typically make?

I would like know what challenges Jan might face in his attempt to lose weight, especially considering the rapid fatigue and drop in glycemic during exercises.
